Submission of locum claims and MO Meeting

Dear Doctors Please send in all your Locum claims by 2nd January 2015. Claims sent after 2nd Jan 2015 will not be entertained. Just a reminder ,Locum Doctors meeting is on 29TH Dec @ 3pm in ED HPP Meeting room. Every klinik Kesihatan must have atleast one representative. The doctors from HPP from the departments that are doing Locum with us must also attend,atleast 2 representative from all disciplines. It will be good if more of you attend. Dear Doctors, We have started few addition slots(11am-3pm, 3pm - 8pm). It will be reviewed whenever necessary. Please be punctual. There will be a Meeting in December for all Doctors doing Locum. Please attend. Thanks

November Locum Request

Dear Doctors, Please take note from October, please put in your LOCUM claim using the new claim form. It is available in the Locum Clinic. Old forms will be rejected. Thanks

Dear Doctors,

Please take note that the locum slots on weekends, the shifts are as follows.
9am -2pm, , 11am-3pm (during  PH only), 2pm -6pm ,4pm - 9pm and  6pm -10pm.

Dear Doctors,

Doctors who change their locum slots with others please remind each other. Few of you forgot and didnt turn up . We are fade up searching for people to replace. Dont be surprise if you are not given slots for few months.  Kindly put in your request for month of September.Please start putting in only at 1pm, names before 1pm will not be entertained.

August assignment Locum 2014

Dear Doctors,

Please make sure those names  on the 5.15pm slot must be punctual to start the clinic. From September onwards the wards doctors will also be given weekend slots. Please dont always request for weekdays. It is difficult to entertain everyone except for ED doctors as they are doing shift.

Please take note some of the doctors have been given 3 slots for July Make sure all doctors turn up for locum. From July onwards those who dont turn up for their  locum will not be given any locum for 2months.

Dear Doctors,

All doctors doing Locum compulsory  have their name chop done. Please contact Dr. Sasi if you need any assistance. Still few writing their MPM No.

Kindly take note all doctors should dress properly when coming to Locum. Eg. short skirts/dress, sleepers and ect  are forbidden. Just to remind  that all of you are doctors ,we are not  coming for fashion show or picnic
