Locum Request for July 2013

Dear Doctors ,

There is a notice  written on the OPD Board. Please read and sign. Only few have read it.
Just to remind all  of you again that if you don't turn up for the  Locum as scheduled, we have decided not to give you  Locum for 2 months for being irresponsible.

Please take note ,all doctors must use the Locum Chop and not your department chop.
