All doctors must get relevant history and document properly . Just a reminder, please use the UPT and urine FEME Strips when indicated only.

Locum request for July 2011
Dear Doctors,,
Put in your request for july. Just to remind all of you that, there are doctors who are not notifying dengue cases coming to the Locum clinic as well as not filling the form porperly. Anyone found not doing so will not be given any locum slot in future. Dengue notification is a must.
