Dear Doctors

Please collect your punch card for locum from the Emergency dept clerk on the 1st of each month.

Take note that Prescription for Syp Promethazine(Syp phenergan) for children below 2yrs is not allowed.

Kindly write the name, I/C and number of days of leave, given properly on the MC books.

Please stamp the carbon copy of the MC. We have problem in exchanging the MC books because it is incomplete.

Dear Doctors,

Please put in your request individually and not in groups. We have being auditing some of the doctors cards , there was no proper documentation. These doctors will not be allow to do locum if they dont improve.

LOCUM REQUEST FOR MONTH OF OCTOBER 2010

Dear Doctors,

Kindly take note that the Locum clinic on weekdays are from 5.15 pm till 10.15 pm. Nobody is allowed to close the clinic earlier without permission. You are allowed to close the clinic only at 9.45pm , on weekdays and weekends.
If there is need to close earlier please inform the Paramedic at emergency department triage and Dr.Elizabeth. You are paid RM 80/= per hour. Please do your duties sincerely.

LOCUM REQUEST FOR MONTH OF SEPTEMBER

Dear Doctors,

Just to inform everyone ,that we will be auditing your OPD cards on your documentation. Those who do not examine patient properly and doesn't write proper history will be barred from doing locum.

Doctors who don't turn up for their locum will not be given any locum slots for the following month. It is really a headache getting replacement.

Kindly see your locum roster properly and dont forget to turn-up. Your co-operation will be appreciated.

LOCUM REQUEST FOR MONTH OF AUGUST

Dear Doctors,

Lately there have been instances where requests for locum slots have been put by other people without that doctor's knowledge. This has resulted in these doctors not turning up for work and we face problems in looking for replacement at the last minute. This practice has to stop immediately. Please kindly put in your request only at 1pm. Any request before 1pm will not be entertained. This is to be fair to those who key in at 1pm as scheduled.

I would like to remind all the doctors that if you haven't registered with us, please come to the Emergency Dept and register.There are doctors who have requested for July Locum and they haven't registered with us. They were not given the locum slot.

LOCUM REQUEST FOR THE MONTH OF JULY

Dear Doctors,

Please put in your request for July. Make sure that all doctors know the date when they are suppose to do their the locum . In the month of June there are doctors who didnt turn up for their locum. It is very difficult getting doctors to do the locum at the last moment. If this happens again I am sorry. that particular doctor will not be given any more locum slots.

LOCUM REQUEST FOR MONTH OF JUNE

Dear Doctors,

Please take note that all instruments used during Locum clinic must be taken care by all staffs. Doctors wii be responsible for the thermoscan and otoscope while nurses will be responsible for the Reflomet. Any instruments lost will have to be replaced by the all the staffs working on that shift.

Doctors must write the medical certificate properly and also document on the OPD card the
MC no. and date.

Please put your request for march. You are allowed to change your request date onces only.
Just to remind again. Please key in only on 15th @ 1pm every month. Any request that comes in before 1pm will not be entertained.
