Locum Request for Jan 2013

Dear Doctors,

Kindly take note that starting from 2013, prescription book will be given to individual doctors to fill the details of patient and the treatment given. Please passover the PS book after your duty to the SN on duty.

All urine FEME must be sent to the main Lab. The urine strip will not be kept in the Locum clinic. Just a reminder again please use the UPT strip only when indicated. Routine Pregnancy confirmation should be done on a normal working days.

All MC should be written properly ,clearly and stamped with your name and MPM no. Dont be surprised if you are not given a slot for the coming month.

Please summit your Locum claims for December 2012 by 31st Dec. Those working on 31st Dec must submit by2nd Jan 2013.

Locum Rquest for December 2012

Dear Doctors,

Please put in your December request. We audited the locum cards twice (Sept&Oct) where we found  few of the doctors have written their signature only but no chop.There were cards not documented properly. We decided not to give Locum slots to those who do not follow rules. Hope all of you will cooperate.

Please post your request . Hope all of you have done your Locum chop with MPM no. Just to remind all of you that if you didnt turn up for the Locum as scheduled, u will not be given Locum slot for the next subsequent month.  Please be more responsible.

Request for the month of October

Dear Doctors,

Hope everyone of you have done your Locum Opd chop. Please get the Opd punch card from the Locum clinic. Just a reminder you are allowed to post your request once only, provided  there is some error in your request. You are allow to put in the request only at 1pm . Please take note if you are doing active or passive calls in the hospital you are not allow to do the locum on the same date of your call.

Locum Request for month of August

Dear Doctors,

 Please take note that you are allowed to post your request only once provided there is a change in your request date.  Any doctors who forget to punch their punch card will have a deduction of RM40 from their claim. The decision made after discussing with the ED adminstrators. We are having alot of problems  with MC books. We are tired of reminding  repeatedly. Those not following the rule will be barred from the Locum. Please chop, fill and write properly.

Locum request for May 2012

Dear Doctors ,

Please put in your request for the month of May. We will be having a meeting with all the Locum Doctors soon. Most probably end of this month. Request all doctors to attend. Inform me if anyone getting 3 slot. There maybe a mistake. It is not easy to do a roster of 90 request. You go crazy. Most of you will get max 2 slot.

Audit of Doctors' Performance for the Locum Clinic

Dear Doctors,

As you are aware, the Locum clinic was meant to reduce the non-emergency workload from the Emergency Dept. It is however administratively managed by the ED. Recently, the number of complaints and problems have markedly increased. Some of the common issues are 1) poor documentation on card and MC books 2) coming late and going off early 3) instructing the counter to close early 4) absent from the clinic without informing and without replacement 5) poor standard of clinical care 6) not following common and routine practice.

As a result of this, we are forming a group to review the performance of all doctors currently registered for the Locum Clinic. This group will be tasked to identify doctors who are not performing well or are repeatedly the source of some of the problems noted above. These doctors, once identified, will be black-listed for a period of not less than 6 months, which means that they will not be considered for the Locum Clinic during that period of time.

The vast majority of doctors are performing very well, but a small number are not; causing much additional administrative work for the ED and causing significant problems for the system. It is our intention to only provide locum clinic slots to doctors who are willing and able to perform the tasks well.

Locum Request for March 2011

Dear Doctors,

We have informed all the doctors many times to write the MC properly and clearly. To be written only by doctors and not by MA/SN. There are few doctors still not following the rules.

ii) Please refer your cases to the correct discipline and admit to the right ward. All doctors must speak to the MO of the concern dept before admitting. Some of the department the Medical officers are on passive call, so call them and discuss before admitting.

iii) All Pediatric cases , please write the weight of the patient on the prescription slip.

Dear Doctors,

There are some changes in the Feb roster. Those doctors who were given locum on 30th and 31st were removed from the locum slot. Very sorry for the mistake. Doctors who are unable to do the locum for the month of Feb, please kindly give your slots to the following doctors. Dr. WangCS, Dr. Dharminy, Dr Mahesh (OPD) , Dr.Farid and Dr. Shamini.
