Dear Doctors,
Please remember that your locum hours on weekdays starts at 5.15 pm till 10.15pm. On Weekends the shifts are from 9am - 2pm ,2pm -6pm and 6pm till 10pm. Please avoid unnecessary cutting of your locum money by hospital accountant if they find that you punch out earlier. Use a separate punch card and also make sure you punch in and out. No punch card which is hand written will be approved.
Kindly notify the dengue cases as instructed by Dr. Teo and fill the form properly especially the contact number and address. Cop and sign.
