Dengue Notification Forms

Dear Doctors,

Please be informed that the percentage of dengue notifications that come from primary care clinics (which includes the Locum Clinic and ED) is currently under intense scrutiny, because it has been included in the KPI at state level. Hence, doctors who work in the Locum clinic are reminded that all suspected dengue cases MUST be notified immediately regardless of whether the patient is to be admitted or discharged.
The process of notification is simple. Fill up the forms immediately and pass it to the Duty Nurse. At the end of each shift, the duty nurse will bring the filled forms to the ED call center and put it in the Dengue Notification Box. We will do the rest.

Failure to comply, has and will, invite the wrath of the higher-ups. Kindly refer to the detailed document and sign when you have read it.

Locum request March 2011

Dear Doctors,

If you have requested for locum slots and need to change the dates again, you are allowed to do so only onces. Anyone requesting more that onces will not be given a slot. Doesn't matter even if your requested dates overlapped with the others. We will try to give you some other dates.
