Dear Doctors,,

Put in your request for july. Just to remind all of you that, there are doctors who are not notifying dengue cases coming to the Locum clinic as well as not filling the form porperly. Anyone found not doing so will not be given any locum slot in future. Dengue notification is a must.

There are some changes in the locum timing. The 3rd MO will come in at 7pm till 10pm on weekdays and weekends the 3rd MO will come at 4pm till 9pm. The locum clinics counter must close only at 9.50pm from June onwards. If any problem please inform me.

Locum Request for the month of June 2011

Dear Doctors,

Please put your request for the month of June. You are allowed 2 slots each. There may be some changes for the 3rd slot for the month of June. Please see the blog at the endof the month.

Just a reminder, please notify all suspeded or admiited cases of Dengue. Full details of the patient name, address and telephone number must be written clearly.

You dont need to notify for Brucellosis cases , But write full details as above in the referral form and refer to the ID Clinic.

Dear Doctors,

There are some changes in the roster for the month of May. We have decided to put 3 doctors everyday due to increase cases of Brucellosis. Only on weekdays the 3rd MO will come in from 7pm till 10pm. Please read the Brucellosis screening protocol which is available in the OPD Locum clinic.

Please take proper history especially consumption of fresh goat's milk and do the relevant investigations. All Bld C&S MUST BE DONE by MO's and not by the SN/PPP. Dont forget that dengue is still our major problem.

Locum Request for April 2011

Dear Doctors,

Please remember that your locum hours on weekdays starts at 5.15 pm till 10.15pm. On Weekends the shifts are from 9am - 2pm ,2pm -6pm and 6pm till 10pm. Please avoid unnecessary cutting of your locum money by hospital accountant if they find that you punch out earlier. Use a separate punch card and also make sure you punch in and out. No punch card which is hand written will be approved.

Kindly notify the dengue cases as instructed by Dr. Teo and fill the form properly especially the contact number and address. Cop and sign.

Dengue Notification Forms

Dear Doctors,

Please be informed that the percentage of dengue notifications that come from primary care clinics (which includes the Locum Clinic and ED) is currently under intense scrutiny, because it has been included in the KPI at state level. Hence, doctors who work in the Locum clinic are reminded that all suspected dengue cases MUST be notified immediately regardless of whether the patient is to be admitted or discharged.

The process of notification is simple. Fill up the forms immediately and pass it to the Duty Nurse. At the end of each shift, the duty nurse will bring the filled forms to the ED call center and put it in the Dengue Notification Box. We will do the rest.

Failure to comply, has and will, invite the wrath of the higher-ups. Kindly refer to the detailed document and sign when you have read it.

If you have requested for locum slots and need to change the dates again, you are allowed to do so only onces. Anyone requesting more that onces will not be given a slot. Doesn't matter even if your requested dates overlapped with the others. We will try to give you some other dates.
