Locum request for March 2013

Dear Doctors,

Please take note that if you have cases that you need to pass over please inform the doctors of the next shift regarding the cases.

From March, all doctors must have their locum name chops . We do not want anyone writing their MPM No. on the card or MC,. You need to use your chops.

Just a reminder ,we need your specimen signature and also your name chop. Please sign and chop on the paper that is available with the staff nurse. There were still MC and Prescription slips which are incomplete.

The Pharmacy dept audited the Locum prescription slips and found that the doctors didn't chop their names  and also there were incorrect drug dosage prescribed. They will reject all incomplete prescription . So follow the rules please.

February Locum request 2013

Dear Doctors,
Pls put in your request for February 2013. Gentle reminder again ,blog will be open only at 1pm on15th of the month . Anyone request before 1pm will not be entertained.
