Locum Rquest for December 2012

Dear Doctors,

Please put in your December request. We audited the locum cards twice (Sept&Oct) where we found  few of the doctors have written their signature only but no chop.There were cards not documented properly. We decided not to give Locum slots to those who do not follow rules. Hope all of you will cooperate.

Please post your request . Hope all of you have done your Locum chop with MPM no. Just to remind all of you that if you didnt turn up for the Locum as scheduled, u will not be given Locum slot for the next subsequent month.  Please be more responsible.
