LOCUM REQUEST FOR THE MONTH OF JULY

Dear Doctors,

Please put in your request for July. Make sure that all doctors know the date when they are suppose to do their the locum . In the month of June there are doctors who didnt turn up for their locum. It is very difficult getting doctors to do the locum at the last moment. If this happens again I am sorry. that particular doctor will not be given any more locum slots.

LOCUM REQUEST FOR MONTH OF JUNE

Dear Doctors,

Please take note that all instruments used during Locum clinic must be taken care by all staffs. Doctors wii be responsible for the thermoscan and otoscope while nurses will be responsible for the Reflomet. Any instruments lost will have to be replaced by the all the staffs working on that shift.

Doctors must write the medical certificate properly and also document on the OPD card the
MC no. and date.
