LOCUM REQUEST FOR MONTH OF JUNE

Dear Doctors,

Please take note that all instruments used during Locum clinic must be taken care by all staffs. Doctors wii be responsible for the thermoscan and otoscope while nurses will be responsible for the Reflomet. Any instruments lost will have to be replaced by the all the staffs working on that shift.

Doctors must write the medical certificate properly and also document on the OPD card the
MC no. and date.

Dear Doctors,

Please take note that any medical officers whose have requested for Locum slots and didnt turn up for duty will not be given any slot the next time. This is really giving us alot of problem getting doctors to do at the last moment.
