Locum Request for August 2013

Dear Doctors,
 We received a complaint from the HOD of Radiology Dept, that our Locum doctors are not filling the particulars on  the OPD card that is attached on the back of the X-Ray form. Please do fill the cards with the name ,sex and I/C.

We have a lot of problems for the past few months, doctors not turning up for their locum slot.  It is a real headache searching for doctors at the last moment.  Please be more responsible.

 My sincere request to all , if you are on leave or not available for that particular month please don't request for Locum . Please be truthful.
